Person Centred Service
At Lavender Group Homes, we adapt our services to meet the user’s needs, hopes and aspirations. Each person will have a person centred plan that reflects the individual’s choices, dreams and aspirations, and each individual’s plan will have meaningful and achievable goals. This person centred plan will be developed following an in-depth assessment, in partnership with the individual, their family and significant others.
We want each person to have as much say as possible in their own lives. We actively promote consultation and involvement in decision making, supporting individuals to express their views and advocate on their behalf if required.
Performance & Quality
We are committed to delivering high quality services and demonstrating continuous improvement. We measure quality by the improvement in outcomes for each person using our service. We seek their views on our service and what we need to do to improve it, which is central to our quality assurance process. To achieve this we employ a range of methodologies which help us identify the views of the individual and other stakeholders and show’s how we are using that information to improve service delivery. Some of the ways we are currently doing this are questionnaires, discussions, local groups and advocacy support.
